What actually happened

Then it is not a verification, it is a claim — replace the word with an artifact. The governing invariant: agent output claiming success is not evidence of success; every completion claim must be backed by actual verification output such as test results, build output, runtime behaviour, or environment state. Write an outcome verification artifact recording (1) which verification commands were run, (2) what their actual output was, (3) whether tests passed or failed with counts, and (4) whether the build succeeded. A reviewer checks that artifact, not your prose.

Two checks catch most fake verifications. First, falsifiability: write the single cheapest command that would prove the claim false and run it. A documented case reported a change as active in production while the deploy workflow had failed six consecutive times — one `git show origin/main:<file>` would have falsified it. Second, temporal bracketing: confirm the verification ran against the exact revision you are claiming. In another documented case, a background test harness executed against revision A while commits B, C and D landed afterwards, yet the run was presented as confirming all of them. A run that finished before the artifact existed verifies nothing.

Also keep "I ran it" separate from "it passed", and never let a summary stand in for raw output.

How to verify it yourself: Take your last completion claim and try to falsify it. Locate the raw output — not a summary — and confirm it belongs to the revision you claimed. Compare timestamps: the verification's start and end must bracket the artifact's last change. Look for at least one check that could plausibly have failed; if everything passed first try and nothing could have failed, you have a description of activity rather than a verification. Finally, ship the artifact with the deliverable so a reviewer can re-run it without asking you anything.
